Clinical Examination 5th Feb 25:

A 9yo pregnant mare was referred to us from a large Veterinary practice with a 4/5 right hind lameness.

Clinical Re-Examination 14 th Apr 25:

9 ½ weeks post initial consultation, the mare presented only mildly uneven at the walk on her right hind
today. There was little digital pulse and no elevated hoof capsular heat.

The surgery site was dry with no smell or exudative discharge.

Photographs show progress of the hoof post-surgery.

28.84mm of dorsal wall growth was measured radiographically since the initial consultation.
The sole and frog were carefully debrided exposing healthy new keratinised organised corium. The sole
has regrown, and new bars have now been established.

The irregular granulation tissue sub solar ventral to the osteomyelitis site has keratinised and was able to
be trimmed.

Radiographs:

Lateral views show increasing sole depth from 3.3mm to 9.02mm and now 12.88mm.

It must be noted; the measurement is misleading as there was no sole initially and subsequent increase in depth is from substantial circumferential wall growth mechanically elevating and subsequently protecting the sole whilst rejuvenation occurs.

No gas lines were detected.

There is increasing sclerosis noted at the solar margin where the osteomyelitis was debrided with incremental P3 margin density noted.

Comments:

The right hind hoof capsule shows no sign of residual infection with radiographs showing evidence of P3 margin resolution.

The capsule has stabilised with approximately 50 – 60% of sensitive laminae wall / P3 adherence in addition to greater than 50% sole/frog papillae adhesion.

The combination of wall and sole growth has been exceptional in just over 9 weeks.

Subsequent to the regrowth, remedial shoeing and packs can now replace bandaging.


Treatment:

The previous surgical site was further resected in addition to the sole and frog.

The aluminium open heeled shoe was re applied post trimming.

A Castle pad was applied then the entire sole filled with Solepac CS. The dorsal wall resection was additionally filled with Solepac CS nullifying the requirement for bandaging.

Clinical Examination:
The colt presented sound with no detectable digital pulse and no capsular heat.

The medial left front quarter crack has grown considerably with approximately 10mm of wall growth from the coronary evident.
The medial quarter is now stable on palpation with no discomfort evident.

Radiographic Examination:
Lateral radiographs show the pedal axis aligned with a mild positive pedal angle.

Dorsopalmar radiographs show the pedal column medial difference reduced from 5.1mm to 1.3mm

The medial P3 avulsion fracture has resolved as shown below.

Comments:
The pedal angle and pedal axis have been corrected with the frog movement now stabilised.

The resolution of the avulsion fracture indicates positive internal capsular resolution.

The quarter cracks have grown down well indicating the capsule is supported correctly.


Recommendations:

  1. Initiate hand walking twice daily for 30mins.
  2. Pack the resection daily with the antibiotic pack.
  3. Re-evaluate in 5 days.
  4. Feed Total Gold at 4 x scoops once per day.

SUCCESSFUL LAMINITIS TREATMENT
HORSE: LEEROY

DATE OF EXAMINATION: 18TH APR / 17TH MAY 24 / 6TH JUN 24
PLACE: EQUINE PODIATRY FACILITY PULLENVALE

Clinical Examination:

LEEROY presented 2/5 lame at the walk left front on the 18th of Apr with a strong digital pulse, heat in the foot and sensitive to thumb pressure to the sole rostral to the tip of the frog. The dorsal hoof wall of the left front has a discharging abscess at the coronary band with multiple wall deficits running down to the solar surface.

Radiographs on initial examination showed extreme active lysis of the solar margin of P3 indicative of a potential osteomyelitis in addition to sinkage and rotational laminitis with multiple gas caps dorsally and ventrally.

The diseased tissue was resected from the coronary down removing all infected material exposing distal P3.

On the 6th of Jun (49 days later) LEEROY presented sound with no digital pulse and no detectable heat. No discharge was evident from the solar or coronary post operative areas.

On re-examination 30mm of new dorsal wall growth was evident as indicated by the red arrow below Fig 3. The hoof capsule was stable with no evidence of further collapse or rotation of P3.

Comments:

Resolution post operatively is encouraging with no recurrence of the osteomyelitis and stabilisation of the hoof capsule.

30mm of new hoof wall growth in 49 days combined with an increase of sole depth of 8.65mm is exceptional (monthly rate dorsal hoof wall growth of approximately 20mm)

Hoof growth is remarkable for 42 days and is critical in sinkage and rotational cases for the survival of the horse


Recommendations:

  1. Continue to pack the operative site daily.
  2. Continue feeding Total Gold at 3 x scoops once per day.
  3. Continue feeding Teff and Bran mash.

SUCCESSFUL PEDAL BONE FRACTURE TREATMENT

Clinical Examination:

  • The filly presented sound at the walk however with a mild bilateral digital pulse. 
  • The left lateral sole has delaminated with healthy new sole re growth.

Radiographic Examination:

Left Front Hoof: Dorso ventral radiographs from today show considerable resolution of the Type IV solar margin fractures evident medially and laterally on the 10th of Jan 24 as indicated by the red arrows below. One medial fracture remains as indicated.

Right Front Hoof: Dorso ventral views show again significant resolution of both medial and lateral type IV fractures. An additional fracture can be identified medially.

Comments:

Fracture resolution times have been unexpectedly short.

Recommendations:

  1. Continue daily bandage changes applying topical iodine as directed for the next 5 days.
  2. Shoe with alloys seated out to circumference without side clips and apply Sole pack CS once soles are hardened.
  3. The filly should remain in box rest walking 20 mins twice daily until shod and re-evaluated 7 days post shoeing.
  4. Continue Bone Gold at 3 x scoops once per day.

HORSE: HERO

Clinical examination on 31st Aug showed skin irritation of the base of the tail, mane and poll. The base of the tail and mid neck dorsal crest shows focal discharging sites of infection.

Re examination on 1st Dec 23 shows resolution of topical infection and hair regrowth over mane, face and tail base as seen in photographs.

History:

The gelding has been treated previously with various unknown corticosteroids that had proved nonresponsive.

Recommendations:

Continue TOTAL GOLD supplementation and daily rugging.

Re-examination of the mare shows considerable resolution of all associated lesions as per photographs. The sub dermal infection has resolved with significant follicular regrowth over the tail base, rump, neck and face as per photographs.

The mare has gained weight, appears comfortable and the seborrheic smell has dissipated.

Comments:

Remarkable recovery from a debilitating chronic skin disease.

Recommendations:

Continue TOTAL GOLD supplementation and utilising the light weatherproof rug.
